All new patients are required to complete the Medical History Form. Please download it by clicking on the links below. The Medical History Form (for both adult and children) can be printed, filled in, and brought with you for your initial consultation. Please fill it out at your convenience, and either submit it via email (info@redhilldental.com.au), or bring it with you to your first appointment.

Many people experience dental anxiety before a dentist appointment. Some of the signs include:

• You feel tense or have trouble sleeping the night before a dental treatment
• You get increasingly nervous while you're in the waiting room
• The thought of a dental visit makes you feel physically ill
• You panic or have trouble breathing when objects are placed in your mouth during a dental appointment


Our dentist will discuss with you the levels of anxiety you may experience during your dental visit. It is important for you to tell the dentist about your feelings, concerns and fears. Our dentist will help you to overcome these feelings by setting the right expectations regarding the discomfort you may experience during the treatment.

Many patients found it helpful to be distracted while undergoing treatment. Rather than listening to the sights and sounds from dental procedures, you can sit back and focus on your music or favourite DVD movies. Our “In-Chair Entertainment” include listen to music or watch your favourite movie - we have more than 40 movies for you to choose from.
